 SUMMARY OF NOTICE OF PRIVACY PRACTICES 

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

 PLEASE REVIEW IT CAREFULLY.
 

The collection, use and disclosure of consumer information is protected by law.  CFS maintains physical, electronic and procedural safeguards that comply with federal standards to protect consumer information.

At no time does CFS disclose consumer information to anyone outside of CFS unless we have proper authorization from the consumer or we are permitted or required to do so by law.

CFS limits access to consumer personal information to those who need to know this information in order to provide treatment/services, payment or health care operations on the consumer’s behalf.

Even if a consumer is no longer receiving services from CFS the agency’s privacy policy will continue to apply to those records.

Consumers may review CFS’s current privacy policy in the agency Policy and Procedure Manual.  Any changes to CFS’s Policy and Procedure Manual will be communicated to the consumer at the annual IDT meeting.

Consumers have the right to request access to their record.  Consumers may make this request to their supervisor.

Consumers have the right to know who their information was disclosed to for the purpose of treatment, payment or health care operations.

Consumers have the right to receive a detailed copy of CFS’s Notice of Privacy Practices.  Consumers may make this request at any time to their supervisor.
